How to Get Care

You can access our services through:

• Direct access. Minnesota law allows patients to receive physical therapy treatment without a doctor’s referral for 30 consecutive days of care. This is called as “direct access.” After 30 days, patients need a referral from their physician to continue care.
• Physician referral. Referral forms are available in our office or online. Click here to download a printable version of our referral form. You can bring this form to your physician or we can fax the completed form directly to your clinic.

Special for Marsh members: All members of The Marsh receive a complimentary half-hour consultation from a Merz physical therapist.

How to Pay for Care

Merz Physical Therapy is a Medicare-certified private practice clinic. We accept most healthcare plans and can accommodate no-fault auto insurance and workers’ compensation cases. We will submit claims on your behalf to most insurance carriers.

We also have very reasonable and competitive prices for patients paying out of pocket. Call our office at 952-935-4037 to determine the easiest and least expensive options for you.
